Katie suggests the intended reader of this sign is someone who wants to be misinformed about their fake pregnancy. With fake pregnancies, I guess, you never do know what to expect.
I actually had a real "scare quote" moment, like, this is the sign in the "waiting room" where you go to see a "doctor" (alien/mad scientist) who will implant you with a parasitic alien.... it's late....
I have no problem with these scare quotes at all. "Your pregnancy" is clearly directed to the couple and/or husband rather than the wife, and "what to expect" would be along the lines of the necessity of restraining said wife from exacting revenge on her husband during the birth.
