There's nothing more absurd than quotation marks around "a", literally nothing :)
I'm kinda hoping lattégirl is onto something. Perhaps there is indeed a phone labeled "A". Otherwise, I can't imagine what went through the author's brain whilst making this sign.
Surely the photographer could have bothered to check whether the alternative telephone in question is marked with an "A"?

They have one of these signs at our local hospital. They do have A, B & C phones. So, very possibly that is the case here.Mary

I think they may be referring to the AiPhone, which is a brand of intercom. I can't say for "sure."
